Certification Renewal

Requirements for certification renewal:
1. This application and a $125.00 renewal fee must be received by the Board of Certification by the last day of your certification term. Please send payment in the form of a check. All funds must be in US dollars and drawn on a US bank.

2. AMI HQ will verify the 3.5 continuing education units (CEUs) earned during the past five years.

Upon receipt and processing of your application materials, you will receive a new certification certificate. Your renewal will be for another five year period.
